Thanks for today's tip on insurance law. I checked the state law, and it requires 30 days' reimbursement for online claims, 45 for mailed. I called the insurance company back, and they certainly started jumping when I mentioned that! Unfortunately, they keep denying my claims based on a mistaken assumption that I need prior authorization for routine outpatient services, which I do not. And every mistaken denial screws everything up all over again!
